Assessing what care you need
Before we can decide what help you can get and how it will be paid for, you need to fill in a needs assessment. This is free of charge.
The assessment helps us understand:
- how safe and healthy you are
- how well you can do everyday things
- how independent you are
- how you feel
- how well you can make your own choices
You can do the assessment for yourself or for someone else. A doctor or care worker can also do it. It helps us see what kind of help is right for you. This could be simple advice or more support.
