Application for a provisional statement
Use this form to apply for a provisional premises licence. This form is intended for premises which are being built or are about to be altered and or extended to allow licensable activities in the future.
Interim Authority Notice
This form is used to allow a premises licence which has lapsed (for example its holder has died) to be reinstated to allow the business to continue trading.
Notification of an interest in premises under Section178
If you are a freeholder or leaseholder who holds an interest in a property, which is the subject of a premises licence, you can use this form to register a notification of interest in the premises so you will be kept informed when changes take place to the premises licence.
Application to renew a personal licence
This form is used by the holder of a personal licence to apply to renew the licence. Personal licences once issued last for ten years.
Disclosure of convictions and declaration
This form must be submitted in addition to an application for a Personal Licence and an application to renew a Personal Licence and allows an applicant for a personal licence to submit a disclosure and declaration of convictions
