If you have any problems completing this form you can contact the Council Tax Office on 0151 606 2002 between 8.00am and 18.30pm Monday to Thursday, 8.00am and 17.00pm Friday and 9.00am to 12.30pm on Saturday.

Enter the Council Tax reference for your property, this is an eight digit number that starts with the number 8.

If the current Council Tax payer is the person who has left, to whom should the account now be addressed?;
If the circumstances giving rise to your entitlement to single person discount occurred sometime ago, you should provide good reason why you did not apply earlier.

If the account name is to be changed, a new demand in the correct name will be issued to make payments on. If the account name is to remain the same, payments should be made in accordance with the demand previously issued until your entitlement to discount is confirmed.
