Care plans
When you contact Social Services, your details are passed to a Social Worker who will visit you to make an assessment of your needs. This assessment will help to determine the type of support that you require.
The Social Worker will then discuss and agree with you the best way to provide this support, and will write a summary of the agreement. This is known as a care plan. The Social Worker will keep a copy and a copy will also be given to you.
