Rehabilitation services

Rehabilitation services is a way of helping you recover the ability to look after yourself and to get you back home if you have been in hospital. People using this service are given help to regain their strength, confidence and day-to-day living skills.

Our aim is to help you to carry on living with as much independence and freedom as possible. We will try and make sure you are given the choices you want. Specialist NHS staff in hospitals and in the community provide rehabilitation services.

Rehabilitation services help people to regain their ability to manage everyday tasks, and return to living more independently, which could be in their home. Wirral Health and the Department of Adult Social Services work together to provide care and therapy in two residential homes for older people, these are Poulton House and Pensall House. This service helps people who might otherwise have to go into hospital, or need to be discharged from hospital early, to receive intensive therapy in the more pleasant surroundings of a residential home. The service aims to help people to return home and avoid the need for long-term care in a residential or nursing home.

If you receive rehabilitation services you can expect the following.

  • You will have an assessment, which will outline your needs and identify a rehabilitation programme
  • You will receive a rehabilitation care programme from a team of fully-qualified staff including a doctor, physiotherapist, occupational therapist, residential care worker and community social worker
  • When you are ready to go home, we will make sure you are fully prepared for this, and you will receive a home assessment visit with an occupational therapist
  • We will explain any charges for your stay

The service will have a time limit and we always aim for you to move to a more independent living environment.
