Assessment of care needs
You will need to have a Community Care assessment before we can consider if you need the help of The Department of Adult Social Services or whether it is appropriate for us to put you in touch with another agency that may be able to assist you. The assessment is very thorough and will identify the areas of need you have. As an example, the questions we will ask during your assessment will include the following:
- What do you think you need help with?
- What are you able to do without help?
- What difficulties do you currently experience?
- What, if any help, do you get now?
- What help do you think you may need?
- Who is your doctor?
The assessment will help us to identify
- What your needs are.
- If you have a carer, what their needs are.
- Whether you are eligible for support from the Department of Adult Social Services.
- We will also provide information about services which may be appropriate to meeting your needs together with details of our charging policy.
The information you give is confidential and will at all times be treated as such, however we may need to share that information with other professionals involved in your care, such as your doctor, or an occupational therapist; should this be necessary we will ask your permission.
When you have your assessment:
- We will consider your views and wishes
- We will tell you what we are doing
- You can have the support of a friend, relative or other person of your
choice during the assessment meeting.
We will ensure that you are able to fully participate in the assessment meeting and if you need services such as signing or interpreting these can be arranged. We can also arrange for documents to be produced in accessible formats. Please make your needs known to the person carrying out the assessment.
We will ask you to sign your assessment and provide you with a copy. We will also provide a copy of the assessment to any other person you have nominated. To nominate a person to receive a copy of your assessment you must advise us in writing of your wishes.
If you are not happy with the out come of your assessment or the way that the Department of Adult Social Services has undertaken your assessment, you can make a complaint. You can talk to the manager of the service who carried out your assessment to try to resolve issues directly, however, if this cannot be achieved you can make contact with the Department of Adult Social Services, Quality Assurance Unit.
