Application for a premises licence

Use this form to apply for a brand new premises licence to authorise one or more licensable activities at a given premise. The licensable activities authorised by a premises licence are: the sale or supply of alcohol; the provision of regulated entertainment and the provision of late night refreshment between the hours of 23:00 and 05:00

DPS Consent Form

Use this form if you are making an application for a premises licence which includes the sale or supply of alcohol. This form must be filled in and signed by your 'Designated Premises Supervisor'. To be used in conjunction with an application for a Premises Licence.

Application to vary a premises licence to specify DPS

Use this form to alert the Licensing Authority that you are changing the named 'Designated Premises Supervisor' of a premises licence.

Consent of premises licence holder to transfer

Interim Authority Notice

This form is used to allow a premises licence which has lapsed (for example its holder has died) to be reinstated to allow the business to continue trading.

Notification of an interest in premises under Section178

If you are a freeholder or leaseholder who holds an interest in a property, which is the subject of a premises licence, you can use this form to register a notification of interest in the premises so you will be kept informed when changes take place to the premises licence.

This form is used by an individual to apply for a new personal licence which allows the holder to authorise the sale or supply of alcohol.

Disclosure of convictions and declaration

This form must be submitted in addition to an application for a Personal Licence and an application to renew a Personal Licence and allows an applicant for a personal licence to submit a disclosure and declaration of convictions.
